The way I see it, here are the possibilities:

1. The owner is too lazy to set it up.
2. The owner doesn't know how to set it up.
3. The owner deliberately doesn't want to set it up, for any number of reasons that are only possible to verify by interviewing all server owners.

I was able to run the CTF enabled map without any problems using the changes mentioned in this thread. Update the attached map file or post what you have in your map options section so we can see what you're doing.

You don't have to worry about world size. You would just have to make your own boundries with a [invisible] mesh surrounding the main area. Once you do that, you'll probably want to make a mesh on the ground outside with death physics, so you don't get people going out there illegally.

Unless they wanted to build their own map around what is already there, they could group the object, and then edit the group properties (shift, rotate, spin, etc.) to put it where they want.
